A diagnosis or hospital discharge does not determine a care setting by itself. These guides cannot assess an individual, determine legal authority, establish benefit eligibility, interpret a contract, or prove abuse or neglect. Verify current state rules, provider records, clinical needs, resident direction, and qualified advice. Use emergency services immediately when someone is in danger.
